Adverse childhood experiences (ACEs) are traumatic experiences in childhood that include all forms of abuse, neglect, or household dysfunction. ACEs can have lifelong impacts on the health and well-being of an individual and their community. In Adams County, 15-20% of people reported a prevalence of 4 or more ACEs. This proposal outlines three effective evidence-based interventions focusing on building early childhood resilience, supporting positive parent-child interactions, and providing tools to community leaders that aim to decrease ACEs’ occurrence and impact and interrupt the transgenerational cycle of ACEs within Adams County.

Reducing the maternal smokingrate, as part of the burden of alcohol and other drug abuse, has been identified as a priority focus for the Adams County Community Health Improvement Plan. Although the county has made some progress in the past several years, the maternal smoking rate continues to be the highest in the state, and well above the national average. A renewed focus on optimizing local community-based resources, rather than reliance on the health systems, and integration of effective systems-level interventions could make a greater impact in this area. As a rural community immersed in poverty, there are many social and organizational barriers to making a positive impact on this public health concern. Through mission alignment and engagement with available community services and resources we have created an evidence-based intervention plan to effectively reduce Adams County’s maternal smoking rate. This plan also includes proven strategies for addressing larger issues of maternal health and resource support in this rural community.

Adams County has expressed concern with increasing vaping/E-cigarette use in middle and high school students. This proposal outlines a project which aims to lower the prevalence of vaping in Adams County teens. It will provide schools with educational tools on vaping, improve students and families’ knowledge of the dangers of vaping, and provide the community of Adams County with strategies to help deter teens from vaping, with the aim to protect the health of Adams County youth.

Adams County Health and Human Services Department is a department of Adams County providing universal health protection for all the members of Adams County, as well as providing essential human services, providing resources to families. Relating to our project, the HHS specifically monitors child abuse and neglect, foster care, and most importantly acts as a resource guide for families. Now, Adams County has faced years of childcare scarcity. Since 2018, there hasn’t been a single inquiry into becoming a regulated care provider in Adams County. Another problem facing Adams County is that the group daycares have closed for various reasons in the past few years (there have been multiple closures, so it is not due to random chance). This is why our group decided to consider hybrid care models as well as increasing inquiries.

Adams County recently hired a new advanced practice nurse prescriber (APNP) into their Healthand Human Services (HHS) Department. The APNP is certified to prescribe medications forconsumers which makes her a valuable resource to the county as other mental health providerscannot. Due to the high demand of this service, the department needs to make decisionsregarding which consumers can be treated internally and which need to be referred to externalproviders. Previously, the county only had the ability to treat adults, but the new APNP isqualified to serve children as well, which will create a massive influx in the number ofconsumers seeking services. The lack of a current standard method for determining whichconsumers to treat internally prevents the department from operating effectively. Uponcompletion of this project, the department will have the knowledge and tools to develop andmaintain a manageable intake process.

The Adams County Department of Health and Human Services has identified a need for finding funding and grant opportunities allocated for health promotion. After identifying gaps and priorities in the county’s Community Health Improvement Plan and Community Health Assessment, three problems areas have been identified as a top priority: Alcohol and Other Drug Related Abuse, Mental Health including trauma and Adverse Childhood Experience, and Transportation. Although the top three priorities have been identified, many other community needs have been addressed in a miscellaneous section including: obesity, nutrition, and unintentional injury.

According to the 2016 Wisconsin County Health Rankings data, there are approximately 3 primary care physicians per 10,000 people in Adams County compared to more than 8 primary care physicians per 10,000 people in the state of Wisconsin. This statistic emphasizes the need for improving access to healthcare, a priority shared by the Adams County community. Our project aims to construct a Community Health Worker (CHW) framework for Family Health La Clinica (FHLC) to implement within the Adams County community. This framework will increase client access to healthcare and overcome barriers that prevent individuals from achieving satisfactory health outcomes. More specifically, FHLC clients will have improved self-management and self-care skills from the health education, interventions, and support provided by the CHWs.

Adams Friendship School District (AFSD) is looking to improve their career exploration program. Some of the main challenges faced within their current program include lack of student and teacher buy-in and lack of student exposure to diverse career options, due to their geographic location in a rural area in central Wisconsin. Both the middle and high schools have time allocated during advisory or homeroom sections each week for career exploration, but these sections are not fully effective due to the issues stated. The district’s career exploration program has had some major changes in recent years, including the implementation of a capstone project in the middle school, so our team focused on enhancing the success of the current program. To spur student and teacher interest and engagement in AFSD’s career exploration program, we created a wide range of deliverables, as well as a list of possible additional ideas to implement in the future.
